Stage A: Learning from Current Practice Performance Assessment

Assess current practice using identified performance measures, either through chart reviews or some other appropriate mechanism.

• Estimated time for completion: 1–2 hours
• Upon completion, credits awarded: 5 AMA PRA Category 1 Credits™

Step 1: Select and register for a live CME activity that has been designated eligible for CME credit toward Performance Improvement. These activities are denoted with a *PI.

Step 2: Download and complete the forms for Stage A

• Course Enrollment Form
• Cover Sheet for Data Collection Forms
• Stage A Credit Form

Step 3: Review the charts of 5 patients who meet the criteria for the specific performance measure listed for the live CME you selected and complete Data Collection Forms for each chart.

Step 4: Submit the forms to the Yale CME office for documentation. Keep a copy for your records.

Step 5: Attend the designated live CME activity. Upon attending the program and completing & submitting the course evaluation form, you will be awarded the AMA PRA Category 1 Credits™ for the activity.
