Stage C: Learning from the Evaluation of the PI Effort

Re–evaluate and reflect on performance in practice Stage B by comparing to the assessment done in Stage A. Summarize any practice and/or outcome changes that resulted from conducting the PI project.

• Estimated time for completion: 1–2 hours
• Upon completion, credits awarded: 5 AMA PRA Category 1 Credits™

Step 1: Download 5 additional Data Collection Forms

Step 2: Conduct a second set of chart reviews to assess the effectiveness of your intervention and complete the Data Collection Forms.

Step 3: Write a short narrative describing your assessment of the success of the intervention(s) you chose and any other reflections on why or why not the intervention(s) had an impact on patient care.

Step 4: Download and complete the Stage C Credit Form

Step 5: Submit the Data Collection Forms, your narrative, and the completed and signed Stage C Credit Form to the Yale CME Office. Keep a copy for your records.
